I don’t know where that was fixed but it was not in the door opening when I pushed the door fully into the wall to be able to remove the debris in the door opening. That was long before Alan was fitting carpet on the landing.  I can’t believe that would have been installed so it was visible in the opening anyway and must have been under the door. If so Alan couldn’t have been able to access the block. 
If you look at the length of the screw used to fix the block to the floor it is only looks about 10mm longer than the thickness of the block. That could pull out of a chipboard floor if the door was stiff when it was opened and closed. 
I would suggest it hadn’t been securely fixed and came loose during normal use.
